I promise to take care during such participation as well as recognize that all activities involving physical movement or development may have certain risks and dangers that could cause injury including , but not limited to, the possibility of severe injury.

I understand that I am solely responsible for any costs arising out of any bodily injury or property damage sustained during my participation in normal or unusual acts associated with the classes selected in this form. I understand that warm up as well as cool down are vital in the development of the class/ activity, and avoiding or skipping warm ups or cool downs can cause long lasting injury.
